New Patient Registration

You may register as a patient by completing the appropriate forms. 

You will be asked to attend to collect the relevant forms and complete the New Patient Health Check Form. You are required to bring identity (i.e. passport, driving licence or birth certificate) and also proof that you live at the address given (e.g. utility bill). Following this an appointment will be arranged for you to register. You will also need to know your NHS number before you can register.

All new patients must attend for a medical with the Nurse.  You will not be fully registered with us until this has been done, although you can be seen for any immediately necessary treatment/temporary resident.  We will only accept patients living within our practice area.  Please confirm this with the receptionist.

Change of Patient Details

If you change name, address or telephone number etc, please call into the surgery to complete a form at reception. If you move outside of the practice area it will be necessary for you to find a doctor in your new area.

The Health Authority (NHS BSC Wales) will help you if you experience any difficulty. The instructions regarding moving to another area are printed on your medical card.
